Registered Nurse ( RN ) Home Care

Divinity Home Care is a private, family owned Medicare A Home Care Agency that operates in Macomb County. We are looking for an experienced RN to join our team to provide patient care and case management alongside our other RNs, PTs, OTs, HHAs, SLP, and MSW.

You make your own schedule each week - so long as patient frequencies are met. Higher weekend rates are offered to provide SOC visits as part of weekend rotation coverage.

Responsibilities:

• Administer nursing care to ill, injured, or disabled patients
• Diagnose and establish patient treatment plans along with case managing
• Monitor and report changes in patient symptoms or behavior
• Communicate with collaborating physicians or specialists regarding patient care
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Maintain accurate patient medical records
• Provide advice and emotional support to patients and their family members

Qualifications:

• 1 year experience in home care setting preferred with knowledge of Medicare A requirements!
• Ability to navigate through user friendly EMR (DeVero) and complete ALL notes within 48 hours of treatment.

Job Types: PRN, Full-time, Part-time

Salary: $40.00 - $65.00 per hour
