This is a mobile positionthat will be traveling up to 50% of the time throughout Maryland to support individuals in the community. This position is not based in Baltimore.
SCI Summary: SCI supports people with disabilities, behavioral challenges, medically complex needs, transitioning youth, the elderly, and other populations using Maryland Department of Health's Targeted Case Management and Supports Planning work models as well as a concierge level geriatric care management and care partner model.
Position Summary: Provide support and direction to supports planners regarding health and welfare concerns of the people they support. Collaborate with MDH and other providers to ensure continuity of care for the people supported. Provide health and wellness education to people supported. Conduct in-person visits as needed.

Essential Duties:
Collaborate with supports planners and SPS leadership to determine those supported who are at risk for health and welfare concerns. Assist with development and implementation of plans to address concerns.
Review medical related Reportable Events and assist supports planners with development of RE Intervention and Action Plans and follow up as needed.
Review Community Options nursing facility referrals and consult with supports planner, nursing facility staff, and family to determine ability to safely transition to the community and have continued needs met.
Conduct in-person visits as needed.
Partner with MDH clinical staff on POS safety issues.
Partner with local health department nurse monitors and other stakeholders as needed.
Determine training needs for medically complex cases, as well as health and safety training needs. Assist with development and facilitation as a subject matter expert.
Develop yearly health and safety promotion and resource calendar, materials, and training for supports planners to provide to people supported on a monthly basis.

Education Required:
Bachelor's of Science in Nursing
Active Maryland Registered Nurse license
Experience Required:
Three to five years nursing experience in long-term supports and services
Experience with psychiatric disorders, intellectual and/or developmental disabilities and substance abuse disorders.

Physical Demands: Generally, presents standard office environment. Must be able to remain in a stationary position >95% of the time. Continually operates a computer, keyboard and mouse, and other office productivity machinery, such as a calculator, copy machine, printer, and mobile Smartphone device. Constantly positions self to comfortably maintain computer equipment. May occasionally be required to move about in an office setting or position office equipment weighing between 10-25 pounds. Vision abilities required by this job include close vision. Ability to operate a motor vehicle. The person in this position frequently communicates with other people. Must be able to exchange accurate information in these situations.
IT Roles must be able to position self to maintain computers and equipment, including under desks and in server room(s). Frequently moves equipment weighing up to 50 pounds.
