Job Description



To provide home and community service care for our company, relevant to nursing interventions required, while promoting and ensuring optimal client care. The position of Registered Nurse shall be a criteria-based position with well- defined responsibility and administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the ADON. Services are furnished in accordance with Agency policies

Essential Duties and Responsibilities
  • Assumes clinical and management responsibility for clients assigned to their care.
  • Assists management in establishing, implementing and maintaining policies and procedures relevant to nursing practice and standards in accordance with local, state and federal regulatory guidelines.
  • Performs a complete physical assessment of patient assigned including history, home evaluation, family support, etc.
  • Assists in evaluating and monitoring the work performance of all home health aides.
  • Assumes responsibility for quality insurance standard relevant to clinical nursing practice for clients under direct care.
  • Assists in the investigation of documentation of all complaints relevant to nursing practice and reports stating complaints to the Director of Nursing or Administrator.
  • Assists in the ongoing staff development of nursing personnel through continuing education.
  • Assists in the development and implementation and maintains an ongoing evaluation relevant to clinical competence for all home health aides.
  • Ensures that nursing licenses, health profiles and clinical skills are kept current.
  • Participates in professional activities and continuing education to include Body Mechanics, Bloodborne Pathogens, Fire and Safety, Infection Controls and BCLS.
  • Ensures that all clinical care medical records are kept current relevant to the clinical care course of the client. This will include, but is not limited to:
    • Plan of Care and Treatment
    • Physician Orders
    • Case Conference Reports
    • Medication Profiles
    • Client Service Agreements
    • Discharge Summary
  • Ensures that documentation in the medical record reflects care provided, client response, educational interventions, and support for financial reimbursement for services/care provided.
  • Ensures compliance with Infection Control Plan.
  • Establishes, implements and maintains direct working relationships with physicians relevant to direct client care issues.
  • Plans with other agencies and develops interagency work and agreements to ensure best coordination and use of resources in the community.
  • Demonstrates knowledge of clinical practice and applies knowledge in client care environment.
  • Demonstrates the ability to effectively verbalize clinical procedures.
  • Ensures that instructions are recorded and maintains accurate and timely clinical observation notes.
  • Performs other duties as assigned.
  • Promotes and maintains an agency environment that is in compliance with federal, state , and local regulations and JCAHO standards.
  • Participates in personal professional growth and development.
  • Gives accurate information to patient/patient's family and home health care staff members regarding cost for home health care services.
  • Uses equipment and supplies effectively and efficiently.
  • Reports to work on time every day on a regular and consistent basis.
  • Performs any and all tasks as instructed by the supervisor.
  • Works any and all overtime when requested.
  • Reports back to work promptly after lunch breaks and/or rest breaks.
  • Performs work to the best of abilities, devoting full effort and attention to the task(s) at hand.
  • Displays an attitude of cooperative compliance in all dealings with fellow members of the organization.
  • Is alert for oppotiunities to improve quality, efficiency or productivity and to bring such ideas to the organization's attention.
  • Ask questions when in doubt as to the appropriateness of any particular course of action.
  • Follows agency Standards of Conduct and Ethical Expectations.
  • There will be heavy client and family member contact requiring strong interpersonal communication skills, both verbal and nonverbal.
  • Strong interface with members of the medical community, home health and HMO/PPO providers.
Knowledge, Skills and Abilities
  • Demonstrates supervisory abilities, good verbal and nonverbal communication skills.
  • Demonstrates working knowledge of Medicare, Part A and Part B, with a strong knowledge base relevant to HMO's PPO's and third-party payers.
  • Strong demonstrated knowledge of the process to include, but not limited to: Assessment; Disease Process; Intervention.
  • Heavy outside interface with physicians, third party payers.
  • Maintains cost effective utilization of client supplies and soft goods and durable medical equipment provided by the agency.
  • Controls all supplies, equipment and personnel assigned to the nursing area.
  • Must be able to meet the physical demands, which includes standing, walking, bending, lifting, and transporting.
  • Must be able to utilize good judgment, demonstrate patience, and maintain a professional demeanor.
  • Must be able to work in a busy and stressful environment.
  • Avoid excessive cologne.
  • Clipped, clean nails
  • No excessive jewelry - watch and wedding band permitted
  • Agency Identification badge
  • Body must be odor free
Education & Experience
  • Class "C " BCLS required; ACLS preferred.
  • Current T.B. screen of one calendar year or current chest x-ray repo1i required.
  • Current CPR certificate or acquired immediately after employment.
  • Annual review of Infection Control Procedures, Universal Precautions, Fire and Client Safety, Body Mechanics, and Blood borne Pathogens is required
  • Six months to one year as a Home/Community Service Staff Nurse preferred.
  • Graduate of an approved School of nursing
  • Licensed to practice professional nursing as a R.N. in the State of Texas
  • Be on the Texas Client' (Client's) nurse aide registry with no finding against the aide relating to client abuse or neglect or misappropriation of client property.
  • Current CPR certificate
  • Current T.B. Screen of one calendar year or current chest X-ray required.
  • Annual review of Infection Control Procedures, Occurrence Reports, instructions on Universal Precautions, Fire and Client Safety, Body Mechanics, and Bloodborne Pathogens is required.
  • The following equipment may be required when making client visits: Stethoscope, blood pressure cuffs, and all therapy related materials as ordered by the physician
