RAYRAC HEALTHCARE SERVICES LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team in Atlanta, GA 30324. The ideal candidate most has experience in Diabetes Control/ Diet and Insulin Management.

Position Details:

• Location: Atlanta, GA 30324
• Job Type: Full-time, Part-time, PRN
• Shift: Night shifts, 
• Days: Friday  8pm - 7am,   Sat  8pm - 6am
• Pay: $33.00 - $36.00 per hour

Key Responsibilities:

• Provide direct patient care in home health settings
• Administer medications and treatments as prescribed by physicians
• Manage diabetes control diet and insulin administration for patients
• Perform medical documentation accurately and efficiently
• Complete evaluations, assessments, and treatments in accordance with policy and individualized care plans
• Monitor basic physical, functional, behavioral, and psychosocial conditions
• Ensure compliance with all regulatory agency regulations and agency policies and procedures
• Participate in ongoing staff training to maintain certifications
• Take patient to all medical appointments as required with followed up recommendations.

Education and Experience:

• Must have an active practical nursing licensure in Georgia
• Experience in home health or group home care settings is preferred
• Experience with diabetes control diet and insulin management is required

License/Certification:

• BLS Certification (Preferred)
• ACLS Certification (Preferred)
• LPN License (Required)

Other Requirements:

• Must be 21 years or older
• Must maintain a valid driver’s license and be insurable under auto insurance
• Ability to pass criminal/registry background checks
• Clear tuberculosis screening
