We are seeking a Sepsis Coordinator for our team to ensure that we continue to provide all patients with high quality, efficient care. Did you get into our industry for these reasons? The Sepsis Services Coordinator facilitates the development, implementation, and ongoing adherence of sepsis program protocols and therapies. The Coordinator ensures the protocols and therapies become the standard of care followed within the facility in an effort to positively impact the overall patient experience.

What you will do in this role:

• Collaborate with medical and hospital staff to provide multidisciplinary care to patients with a diagnosis of sepsis.
• Round with physicians with diagnosis of sepsis and review clinical data.
• Provide education and clinical expertise to care team, patients, and families.
• Collaborate with physicians and staff in an effort to decrease mortality and length of stay.
• Identify and integrate patient learning needs, abilities, and readiness into the Plan of Care.
• Work in collaboration with the nursing team to develop and implement individualized teaching plans for patients and families in regards of Sepsis diagnosis.
• Collect and analyze sepsis program patient data to assist with documentation, clinical care, and improved outcomes.
• Participate in rounds and proactively respond to patient needs to improve patient outcomes.

What qualifications you will need:

• Associates Degree in Nursing or RN Diploma required; Bachelors Degree in healthcare related field preferred.
• Current licensure as a Registered Nurse (RN) in the state of Tennessee or compact state required.
• 3 years of related clinical experience required.
• Must possess a knowledge of systems understanding, performance improvement tools/methodology, and facilitation/teaching skills.
• Current BLS required.
