Under the direct supervision of the Registered Nurse (RN), the Licensed Vocational Nurse (LVN) provides intermittent skilled nursing services to patients in the home setting in accordance with the patient’s Plan of Care. Participates in the planning, evaluation and implementation of individualized patient care in coordination with the RN.
QUALIFICATIONS:
A. Currently licensed as a Licensed Vocational Nurse by the State Board of Nursing;
B. One year prior nursing experience preferred;
C. Ability to work in a field setting and exhibits ability to make sound nursing judgements;
D. Effective communication skills;
E. Have and maintain an automobile to be used for work;
F. Is able to meet physical demand of standing, bending, lifting, stooping or performing other work requiring light physical exertion (up to 30 pounds) on a continuous basis (over 50% of time); or moderate physical exertion (30 to 50 pounds) on a frequent basis (16% to 50% of time); or heavy physical exertion on an occasional basis (up to 15% of time).
DUTIES:
A. Provide intermittent and skilled nursing services including assessment, evaluation, procedures, teaching, and training activities as outlined in the patient Plan of Care.
B. Report significant findings to patient’s physician and RN as they occur.
C. Submit completed skilled nursing notes, and communication notes per agency policy.
D. Submit change orders to RN Case Manager.
a. Schedules a case conference with assigned RN to review patient’s needs, problems, level of care and any changes in Plan of Care of next certification period.
b. Completes communication notes documenting plans of recertification discussed and agreed upon between the physician, patient, RN and RN Case Manager.
E. Participate in updating the Patient Plan of Care on an ongoing basis.
F. As applicable, participates in performance improvement programs through submission of data collection as it relates to direct patient care problems and serving on Performance Improvement teams.
G. Follows agency policies and procedures.
H. Attends staff meetings and educational in-services per agency requirements.
a. Continually strives to improve nursing care by broadening knowledge through formal education, attendance at workshops, conferences and participation in professional and related organizations and individual research reading.
b. A minimum of 50% of yearly in-services.
I. Effectively communicate with all members of the multidisciplinary team, the patient and physician.
J. Acts as the patient’s advocate and as such, is liaison to assist in communicating the patient’s needs to the multidisciplinary team.
K. Participate in the discharge planning process.
L. Documents discharge planning on skilled nurse notes.
M. Perform other duties as assigned.
